A damaged surface, already solved.

Screen Assurance is a month-to-month replacement program for Qualia Forge projection surfaces. For a flat monthly rate per square foot, a surface damaged in service is repaired on site where practical, or remanufactured to its original specification and delivered to the venue. No deductible, no claim paperwork beyond photographs and a return.

How it works

Enroll

Surfaces enroll at delivery, or later after a condition review (photo or in-person). Late enrollments carry a 14-day waiting period. Each agreement lists its surfaces on a schedule with dimensions, family, and rate.

Pay the month, hold the coverage

Billing runs per calendar month, invoiced ahead and due before the 1st. Payment received in full activates that month. An unpaid month is simply not covered; coverage resumes with the next paid month.

Report damage

Notify claims@qualia-forge.com within 7 days of discovering damage, with photographs and the agreement number. We assess remotely, and where practical resolve with an on-site repair.

Repair or replace

If the surface cannot be restored to published specification on site, we issue a return authorization and manufacture a replacement to the original spec. We cover standard outbound freight in the contiguous US; the damaged surface returns to us and becomes our property.

What is covered

Covered

  • Accidental damage during normal exhibition use and handling at the enrolled venue: tears, punctures, impact damage, edge damage.
  • Staining or marking that cleaning cannot restore, including damage from a good-faith cleaning attempt.
  • Delamination or coating damage caused by impact or handling.
  • Damage caused by third parties at the venue (guests, crew, other trades).

Not covered

  • Loss or theft: a claim requires the damaged surface back.
  • Willful damage by the client or its contractors.
  • Fire, flood, and other catastrophe events (property insurance territory).
  • Damage in transit arranged by anyone other than Qualia Forge.
  • Alterations: cutting, re-grommeting, coatings, or repairs by others.
  • Use outside published specification, including outdoor exposure of indoor-rated surfaces.
  • Frames, rigging, and hardware (covered by their own warranty).

Billing, in practice

Each enrolled surface bills at its family rate times its area, with a minimum of $50 per surface per month. The first month prorates daily from the coverage start date. Invoices issue around the 20th for the following month and are payable by ACH or check; a 5-day grace period applies before a month is treated as lapsed. Once payment posts, a Certificate of Coverage for the period is available on request. Optional service program fees of this kind are generally not subject to California sales tax; confirm treatment with your tax advisor.

Enrollment quotations, monthly invoices, and certificates are produced with the Qualia Forge assurance billing tool, referencing one agreement number throughout.

1.The Program

The Qualia Forge Screen Assurance Program ("Program") is a commercial service agreement between Qualia Forge ("QF") and the client named on the agreement schedule ("Client"). Under it, QF agrees to repair or replace enrolled projection surfaces damaged in covered circumstances, in exchange for a monthly fee. The Program is a product service agreement. It is not a policy of insurance, and QF is not an insurer; QF's sole obligation under the Program is the repair or replacement of enrolled surfaces as described here.

2.Definitions

"Surface" means a projection surface manufactured by QF. "Enrolled Surface" means a Surface listed on the agreement schedule. "Coverage Month" means a calendar month for which the fee has been received in full. "Venue" means the installation address listed on the schedule for an Enrolled Surface.

3.Eligibility and enrollment

Only Surfaces manufactured by QF and installed per QF guidelines are eligible. Surfaces may enroll at delivery with no waiting period. Surfaces already in service enroll after a condition review (photographic or in-person), and coverage begins 14 days after enrollment. QF may decline enrollment of a Surface based on age or condition; Surfaces are ordinarily eligible through their fifth year from manufacture, extendable at QF's discretion.

4.Fees and billing

Fees accrue per Enrolled Surface at $1.00 per square foot per month for 100% Opaque (OP) family Surfaces and $1.50 per square foot per month for Acoustically Transparent (AT) and Transparent Scrim (TS) family Surfaces, measured on finished visible area, subject to a minimum of $50 per Surface per month. The first month prorates daily from the coverage start date; thereafter billing is by whole calendar month. Invoices issue in advance and are due before the first day of the Coverage Month, with a 5-day grace period. Fees are exclusive of taxes, where any apply.

5.Activation and lapse

Coverage exists only for Coverage Months. Damage occurring, or first discovered, in a month that was not paid is not covered, regardless of payment in earlier or later months. After a lapse of 60 days or more, re-enrollment requires a new condition review and the 14-day waiting period of Section 3.

6.Covered damage

The Program covers accidental physical damage to an Enrolled Surface occurring during normal exhibition use, cleaning, and handling at the Venue, including tears, punctures, impact and edge damage, staining that professional cleaning cannot restore, and impact-related delamination, whether caused by Client personnel or third parties present at the Venue.

7.Exclusions

The Program does not cover: (a) loss or theft; (b) willful damage by Client or its contractors; (c) fire, smoke, water, flood, seismic, weather, or other catastrophe events; (d) damage in transit not arranged by QF, including Venue relocations; (e) alteration of the Surface, including cutting, re-grommeting, applied coatings, or repair attempts by parties other than QF; (f) use outside QF's published specifications, including outdoor exposure of indoor-rated Surfaces; (g) frames, rigging, mounting hardware, and any equipment other than the Surface itself; (h) cosmetic wear that does not affect projected image performance at the Surface's published viewing conditions.

8.Claims

Client must notify QF at claims@qualia-forge.com within 7 days of discovering damage, including photographs, the agreement number, and a description of the event. QF assesses each claim and may elect: (i) an on-site repair, by Client with a QF-supplied repair kit or by a QF technician; or (ii) replacement. QF's election is made in good faith against the standard of restoring the Surface to published specification.

9.Replacement, return, and allowance

Replacements are manufactured to the original specification (or the nearest current equivalent if the original is discontinued) with a target lead time of 10 to 15 business days plus transit. QF pays standard outbound freight within the contiguous United States; expedited or international freight is billed at cost. Upon replacement QF issues a return authorization; the damaged Surface must ship to QF, freight at Client's cost, within 30 days of delivery of the replacement, or the replacement is invoiced at the then-current list price. Returned Surfaces become QF property. Each Enrolled Surface is allowed one replacement in any rolling 12-month period; further replacements within that period are offered at 50% of list price. On-site repairs do not count against the allowance.

10.Term and termination

The Program runs month to month. Either party may discontinue with notice before the next Coverage Month; fees for commenced months are not refundable. QF may terminate an agreement for misuse, misrepresentation of damage, or repeated claims outside the scope of Section 6.

11.Rate changes

QF may adjust rates with 60 days' written notice, effective for Coverage Months after the notice period. Continued payment constitutes acceptance of adjusted rates.

12.Transfer and relocation

Coverage attaches to the Enrolled Surface at its Venue. Transfer to a new owner, or relocation to a new venue, requires written notice and QF consent, and may require a new condition review. Transit for relocation is not covered by the Program.

13.Limitation of liability

QF's entire liability under the Program is the repair or replacement of the Enrolled Surface. QF is not liable for consequential, incidental, or indirect damages, including lost revenue, lost bookings, or cost of substitute equipment. In no event will QF's aggregate liability for an Enrolled Surface exceed the greater of the fees paid for that Surface in the prior 12 months or the list price of the replacement Surface.

14.General

The agreement is governed by California law. Any dispute will be resolved by binding arbitration administered by JAMS in Los Angeles County, California, except that either party may seek injunctive relief in court. If any provision is unenforceable, the remainder stands. QF is excused for delays caused by events beyond its reasonable control. The agreement, its schedule, and these Terms are the entire agreement regarding the Program. Notices go to the addresses on the schedule and to info@qualia-forge.com.

Acceptance

Executed by the parties as of the coverage start date on the attached enrollment schedule, which lists the Enrolled Surfaces, rates, and monthly fee. Agreement number: ____________________
